Is the hose size 5/8" ID?

Yes, the coolant hoses are 5/8”.

Wondering if the heat exchanger would need to be unbolted if I need to replace the old coolant hoses connected to it.

I would not disturb the heat exchanger if I didn’t absolutely have to. With the 4x4 you may not be able to get to them from below. Mine was RWD.

No doubt. Just considering it as a preventative measure. Not sure if they have ever been replaced.
