This is the template I used to make my freeze plug plate. I got the bolt hole measurements from JWilliams3 and then printed this picture using paint and margins set to 0.18". Once it is printed, I checked that the measurements are accurate with the picture, then traced the area I wanted the plate to cover and cut it out of 1/2" plate. I drilled 9/16" holes (the bolts are 12mm x 1.75) so there was a little room for play so all the bolts lined up. Hope this helps someone. The bolt hole dimesions are as follows:
top hole across 7 11/16"
bottom hole across 7 15/16"
top to bottom hole 2 3/8"
*All measurements are center of the hole to center of the hole.
