Not too hard. The worst part is getting the old ones off. If you have a small pulley puller sure helps. Other then that it's pretty straight forward. If you get into it and need some info don't hesitate to ask.
You might consider the Pacbrake. A very amazing product! I drove an '06 IIRC that had a Banks brake, boy was that thing a waste! It sure didn't brake near like a Pacbrake did.

Sounds like you are thinking down the right track. If that doesn't fix that problem then you will be looking at a rebuild. The stock bands are known for burning up. When and if you get it rebuilt make sure you get a Kevlar band, they don't wear out like the originals.
No. You will have to remove the transmission to replace the band. And unless you are good with trannys take it to a reputable rebuilder. Is the transmission giving you any problems?? If not I wouldn't worry about it. I would recommend tho while you are into it, you should adjust the bands.
I find that true on my truck to, altho I have stock injectors, it's so clean. At least in high elevation(4500'), but in low elevation it'll roll the coal till the turbo lights then it cleans right up.
