I had a similar problem on my 03 2500. I had a fuel rail leak and my fuel pump was original at 215K miles replaced both and truck runs like new for 2000 miles now .I however never got a code. I had the Dealership fix it#@$%!

First they tried just relaxing the fuel pump, test drive it with no improvement so they kept working toward the engine block. Took off the fuel injection pump and I don't k ow what they saw but something made them realize it was the original pump so they replaced it and also saw a small leak...

So it turns out i was running on the Original Fuel injection Pump, It lasted 214000 miles... and the Limiter valve would leak around 16000 psi. So a replaced Fuel Injection Pump and injection manifold and the truck is running smoother and sounds much better. Thank you

Off the fuel filter? No the only thing I see coming of the back of the filter canister was a line that I followed back to the fuel tank. So I think the pump is in the tank, like I've read.... I had to take it to the dealership, so hopefully I'll get a call from them today.
