03 off-color smoke at idle and on decel

I have an 03 with manual trans and 167k miles and recently it started smoking at idle hot or cold, I am Cummins trained and have access to a variety of equipment but not a DRB tool - so I did a cutout test with a SNAP On MODIS and 2 and 6 were the smokers. I removed all 6 and did a compression test to eliminate any surprises, put in 6 BOSCH reman injectors and I still have smoke on 6 - I swapped 6 and 4 around and the smoke is still from 6. I then hooked up a 4-channel scope to the injectors and found that 6 is being fired out of time compared to the others - has anyone ever heard of this? Lift pump pressure is normal and I also checked for a drain line restriction normal also. Thanks, Mike.
