On my dads 1993 w250 the voltage meter fluctuates way more than it should and all of the lighting pulsates with the voltage meter. It has a new alternator and headlight switch. Also when he hits bumps in the road the lights temporarily go off and come right back on. The dash lights do not work but the tail lights do, If any one can help me figure out where to start, it would be a huge help.
 
Sound's like you have a Ground wire problem . Check the wiring across the core support there is a ground wire right by battery. And also check the ground wire at back of Motor on firewall .If they are both tight then look under dash by ash tray behind that their is 2 ground wires hooked to dash .
 
Mine did the same thing. I took the pcm loose and sanded mounting locations. This fixed the charging issue. My headlights going on and off was a defective " new" headlight switch.
