That is weird..... possibly a bad map sensor? Have you checked for codes?

i would agree... map sensor... . may or may not set a code

turn the key on and off 3 times and on the 3rd time leave the key on ( don not start) and read the codes on the OD window

You really can't clean a map sensor but, I would at clean the IAT sensor or better yet, replace the IAT and Map sensor since when these two start to go a little crazy the give poor MPG as a symptom. Also, please give us more info on your friends truck. Milage, mods if any, stick or auto?

I've heard of a bad lift pump causing low mileage but still allowing the truck to run, check his fuel pressure, after that replace the IAT.

I just bought a brand new Map sensor on ebay for $10... ... maybe watch ebay & save some cash on replacing map & IAT.
