I thought it has been working for the last 1/2 of a year(since I have bought it) but today i actually had to park on a hill and she just kept rolling backwards. THe truck is a 01 4x4 6 speed with rear discs. Is there an adjustment or do i need ot replace something?
 
Mine wouldn't hold on an incline either. It's simple and I really shouldn't have waited as long as I did to adjust it.



Take the rotor off and you'll see the e-brake shoes. Turn the star wheel adjuster (top center in picture) to spread the shoes apart. I turned the star wheel 6 notches and it'll now hold it great. Adjust both sides. On the passenger side the star-wheel is on the bottom. I took some pictures when I adjusted mine since I had not been able to find any others on TDR and wasn't sure what to expect... but hadn't gotten around to posting them. Hopefully the picture will attach and give you an idea of what to look for when you get the rotor off.
 
You don't even need to take the rotor off. There are slots in the back cover that have rubber plugs in them. Pop the plugs and you can get to the starwheel with a flathead screwdriver or a drum brake adjustment tool (like $5 at your local parts store). Jack up the wheel and spin it while adjusting the starwheel. When you start to feel drag from the shoes, stop and give the starwheel a couple of flips in the opposite direction 'till the drag goes away.

There's a turnbuckle on the rear cable to the e brake attachment. Tighten and enjoy.

Sure looks like a caliper mounting point at the 2 o'clock position to me...



And some really small diameter shoes (smaller than the hub) and a really big dust sheild (roughly disk brake sized).



The turnbuckle on the cable is to adjust the cable slack. The starwheel is the proper way to adjust the brakes.

Guess I should have given a better description of the picture. I do have an 02 with disc brakes... . yeah I know where's my profile :) That picture shows the e-brake drum shoes after I have pulled the caliper and rotor off. I did see the rubber gromet on the rear of the hub but was concerned that if I pulled it out I wouldn't be able to get it back in and then the salt, sand, and other road grime would get in... but I'm sure it would have anyway. Hope this helps.
