no communication between ecm and injection pump. Sign that the pump is on its way out.
Could clean the plug at the ecm and injection pump. Blue Chip Diesel has a good troubleshooting procedure for the pump.
 
Like I have said before 1689 code does not always mean a bad VP. my 02 kept coming up with the 1689 code & I took it to Dynamic Diesel in Reno,NV & they cleaned all the connections, VP, ECM, PCM, PDC & Banks. I had already cleaned both battery terminals. Since that was done I have not had anymore problems. That was around 20,000 miles ago. The tech said that on these older trucks the connections have a tendency to get dirty. He also said that they have to be squeaky clean. This may be your problem. Better to check then replace & VP that might not be bad.
