I've seen it done on a Ford. The guy wanted a F350 4X4 Crew Cab dually but they didn't offer it when he bought his. I'm not sure what it cost him. I do know that the thing eats tires. He's had the thing to 3 different shop an all of them tell him that the conversion kit causes the problem. He's got over 100,000 on the truck with the dually set up.
Had a buddy recently inqire about this very thing! Dually kit, installed and painted: $4000... in the end, not really worth it given the price differences between trucks in any given model-year.
There are a couple places in Trailer Life magizine. Have seen several in campgrounds, on Dodges and Chevy Surbans, and Fords Not one complaint that I've heard.
