I think the 3rd syncroniser in my truck is gone. After I put the dual disk clutch in about 5000mi ago this gear had always seemed more notchy than the rest. Occasionally it would grind but not a big deal, but now you can't down or upshift unless the rpms match the driveline speed or it grinds until they finally mesh. How big of job am I looking at to put in new syncros. This is still a nv4500 five speed. I have heard about the fifth gear nut but other than that is there anything else i should address when I tear it down? :(
 
Sounds like your dual disk clutch is not releasing completely. I'd check that when you tear it down. This would account for the notchy shifting and synchro failure. Even if your synchro is gone, it should shift, albiet poorly, assuming complete clutch release, without having to match the engine speed.

If the clutch wasnt disengaging wouldn't that mean the other gears should be showing some of the same signs? They shift as good as they ever have, it is just 3rd gear
 
Let me guess: You start in second and jam it fast into third to get out of the intersection, then by the time you shift to fourth or fifth you are not in such big hurry, so you shift a little slower? That would explain why 3rd went first. Change the 4th synchro also. First and fifth will probably see the least wear.
