One can lock the 5th wheel hitch to the bed rails, but the nuts for the bed rail bolts are exposed under the truck bed. How does one "lock" or prevent someone from just unbolting and removing rails, hitch and all?

You really can't. Plus there torqued pretty tight. If somebody wants to steal it, it would be easer to use a torch and cut the locks, which would take about 30 seconds.

Few theives would work that hard!

Having delt with a few thousand, I find them to be oportunist--not workers.
