does anyone know who makes a replacement for the plastic inserts that go into the door to mount the large towing mirrors i think they are called west coast mirrors?
 
I don't, but to help you search, those mirrors found on pickups that mount to the door skins are usually called either Low-Mount or sometimes Four-Point mirrors.*
West Coast mirrors are the style like you see on big-trucks, that mount to the top of the door frame above the window and to the door just below the window.*
 
If you are talking about the coated rivnuts in 1/4 20 they were also used on mid70-80 gm pick-ups.Chevy sold them for less than Dodge back in the day(.this may or may not still be correct)They are still available from GM. That's what mine were in my '98 but the OP insists his are plastic.

I was going to suggest a well nut, just need to know hole size and bolt size. Well nuts are used on our school buses for the crossing mirrors and they hold very well, even with kids swinging on them.
