Engine/Transmission (1998.5 - 2002) 99, Miss At Idle, Codes

Got a 99 in the shop, misses badly at idle and goes away as rpms go up. Can't feel it past 1200 rpm. Have codes P0216, p0217, p1693, p1689. Lift pump pressure was less than 5, so I replaced pump and filter, problem still persists. Injection pump is probably bad because of p0216 code, but I'm wondering if it may be a injector problem instead. Any idears?
