Has anybody added a Banks ram air scoop (part # 42190) onto their truck to flow into the stock airbox? If yes, what steps were needed to accomplish this?

Yep seen them. I think it is a terrible idea. Every bit of road dirt will be forced right into your filter. In the winter every bit of snow and ice will be forced in there. In the rain same thing..................
 
You could do something similar with some PVC pipe for a couple of bucks, and either leave it a little shorter or spin the downturn backwards so its not funneling all the elements into the box
 
Then it wouldn't be ram air................

I think there are very good reasons the OTR trucks have large elements high up on the trucks.
 
Unless your shooting for high horsepower, the factory set ups are quite sufficient. Its been proven countless times on the dyno that CAI make very little difference.
 
The one difference it does make is in EGTs. I get a legit 25 degree drop towing up a 6% grade with my Home Depot airbox mod. 25 degrees for ~$5 and 15 minutes work is a great bargain for me. As you said, no noticeable power increase though.
