They changed the headlights & the grill. They also changed the dash. Pretty much the same changes that took place between 2nd & 3rd Generations.
 
Short answer is "NO"...



'03's made significant engine changes, as well as suspension, interior and sheetmetal - other substantial drivetrain changes soon followed. But there were FAR more upgrades done between '02 and '03 than between '05 and '06...
 
03 was a completely new truck from 02. Of course some driveline components carried over, but vehicle wise an entirely new design.

The transmissions and transfer cases.



Not only did the engine get HPCR for '03 there were quite a few internal changes too, including a new block, bedplate, head design, fractured-cap rods, etc.
 
The NV4500 5-spd was also a carryover in the 3rd gen. SO trucks untill "04. 5" when the 325/600 HO come out, certified in all 50 states.

Its what the industry like to call a refresh, then the full-on redeisgn will come in like 2009 - Product cycles are becoming really short to keep customers coming back, But then even 4 year old cars look dated... :(

Actually the SO's lived in California and several eastern states cuz, HO's were not allowed. Not until 04. 5 did they go away.
