I just noticed page 14-63 of service manual, where they warn you in bold text that a bad fuel heater/thermostat (300 Watt) will cause fuel waxing and smoke/ poor running in cold weather and will NOT set any codes since it is not controlled by ECM.



Just a heads-up for those with the symptoms.
