From your title, I assume you are not the owner of said truck?

Did you check the body mounts? Up here in NY they rust out on the oldies and the cab will drop about that much (speaking from experience here, and is the reason why I have a '99 listed below) , I don't know about the fender(s).

Is it a 2 or 4 wheel drive? Have you checked all the ball joints or king pins on the knuckles or control arms, or whatever it is that holds your tire and suspension on (sorry I have nothing but 4-bys in my family)?
 
From bilt More info: Sorry guys the truck is 1992 2 wheel drive standard cab stock truck, with a 10" Alaskan camper. New front springs & shocks. The alignment shop says its looks OK ? With the truck empty its still low on the drivers side. Can you put 350 springs in a 250
front end?
 
There was a TSB issued by Dodge for a spacer on the drivers side coil to level things out. Also the HD D350 coils will fit in the D250 and will perk the front end up nicely, I did it on my Dads 250.
