Once the plastic valve "cover" is removed, it is only two screws that hold the CCV in place. That being said, it DOES need to be pried out with a flathead screwdriver or prybar once the screws are removed, becuase it is a VERY tight interference fit with the thick o-ring that seals the CCV to the aluminum valve cover.
FYI-- on the 5. 9 CTD, the CCV is NOT a scheduled replacement item--it ONLY needs to be replaced if it is leaking around the valve cover or near the hoses, and MOST times those leaks can be fixed simply by replacing the o-ring or cutting back the hoses and adding small hose clamps to the connection... no need to buy the whole new filter, which is a simple baffle box to direct oil back to the crankcase.
The 6. 7 CCV is a different beast altogether, that DOES have a scheduled replacement interval.
