I have a 2005 CTD which has gradually picked up an occasional rough smokey start, a knocking-like sound when warm at idle, and some knocking vibration at around 2000 rpm in the shifting lever. I first thought it was an injector issue. However, I also noticed that one or more of the injector lines had a bit of dust near them as they entered the head. Moreover, the common rail manifold also had a thin coating of dry dust which smelled like diesel when wiped off. When I took the truck in for service, I pointed out the dust to the mechanic and mentioned the knocking issue. He checked the injector lines and all six were loose! You need to tighten both the inner and outer fitting on each cylinder line (as they enter the head). A loose inner fitting will cause an internal leak (not visible) and possibly a no start condition. A loose outer nut creates visible leaks. After the fittings were tightened, all the strange behavior appears to have gone away. Apparently loose high pressure fuel fittings from the factory are not that uncommon. You might want to have your's checked as a precaution. It was a quick and easy fix for my truck's problems.
