All of my Ram duallies have been hub-centric. The tapered lug nuts you show are for lug-centric wheels.

I used to think that. When I went to buy steel wheels for some Winter tires (for a Hyundai) I discovered that OEM wheels are always hub-centric (w/ tapered nuts) and [cheap] generic wheels are usually lug-centric (w/ tapered nuts). And my dually is hub-centric with flat nuts.
 
I have factory wheels on the inside and ultra aluminum wheels on the outside . I am beginning to wonder if I was sabbatoged I will post pics of the lug bolts that came from the wheel that fell off threads looks fine . I see the owners manual says to check the lug nuts at regular intervals so I bought another torque wrench to keep under the back seat so I always have one with me I can check while fueling up or what ever

The reason flat lug nuts are used on hub-centric wheels is that, if the hub OD and the lug circle are not absolutely true, a tapered lug nut can place one or more lug studs in a bending moment as it forces the stud to center in the tapered lug hole in the wheel. The flat lug nuts allow the wheel to center on the hub without imposing bending forces on the studs. Using both a tight hub-centric fit with tapered lug nuts, from a design perspective, doesn't make it right.
