My '99 has 140K miles and is soon to get a new SB ConO, including TO brg, flywheel, and pilot brg. Clutch hydraulics replaced about 50K ago with stock system. Truck is used pretty light duty, seldom loaded, seldom pulling, no racing.

What are the extras that go with this job? Rear main crank seal, front trans seal, trans input brg?

You already have all the common stuff to replace when changing the clutch. The other things you listed would only be for preventative causes unless they might be bad. I assume you have the 5 speed being a 99?
 
I know you said the hyr. were replaced with stock, but you might consider SBC's adjustable hyr. system. I didn't put it on at first, but found after my Con-OFE was installed that the throw from the stock set wasn't enough. Shifting was hard to do as the clutch wasn't disengaging properly with the stock hyd. This isn't the case for every truck or clutch. My buddy's 99 could use it, but it is liveable with stock. Mine wasn't. Now that I have the adjustable hyd. all is great. Easy to adjust, too. Just turn a rod under the dash right behind the pedal. I think retail is about $250. You may want to try the stock first and then change it only if you need to.
