2nd Gen Non-Engine/Transmission Convex mirror replacement

Yesterday the convex mirror on the drivers side fell out and broke when my wife shut the door. It appears that the little piece of tape and the silicone dried out. Does anyone know where a replacement can be found or is it a dealer item?

The first modification I made to my '95 was to get the fun house mirror replaced with a flat one. The local glass shop cut me a flat mirror, removed the old one, and installed the new for $15 parts and labor. Didn't take long to do. That was 240K miles ago.
