Diesel Shop near Middletown, NY

Looking for a diesel shop near Middletown, NY. I have the xxx amount of miles until restricted to 5 mph, Service DEF system, see dealer.

What happened to cause the de-rate? Truck type? There might be more help with more details. I had that happen twice, failed DEF pump and a stuck code that wouldn't clear. Pump was on a trip were they also cleared all codes. 3 weeks later it popped again and took to dealer and they cleared, came back with nothing else on their computer. Cleared it again and it's been good.
