I have had this engine out twice i replaced both the rod bearings and main bearings. everything was really tight and in spec. valve lash is . 010" intake. and . 020" exhaust. pistons are in great shape, cylinders still have a cross hatch. This is a 2001 engine with 140k miles with 5 speed. Has anyone ever had a noise like this?? you can hear it clicking when you crank it before the truck starts. I just replaced the injectors with known good injectors too.

I'd have to guess it's in the valve train or the valves. Disconnect the ECM power, pull the valve cover and have someone crank it while watching the rockers. Could be a bent valve. Did you check the head when you had it out?
 
You have a miss fire for sure along with that noise so I would agree that you have a valve train issue. Bent valve, one adjusted too tight maybe? When did the noise start, was it after working on it or did it just happen one day? Crack the injectors one by one while its running and see if you can narrow it down to one cylinder, then take a close look at that one. Maybe pull the rocker assembly off and make sure the valves are the right hight and check the push rods.
 
Why did you have to replace the bearings?

Are you sure all cylinders are firing? When I had my split injector tip it sounded like that. I would double check by cracking injector lines one at a time to see if you can find it there. Then I would do as suggested above and go through valve train. The only reason I say that is because I know what a PITA that valve cover can be. I can't believe you pulled the motor twice!
 
well I got screwed over when I bought the truck. the po band aided a cracked 53 block that was cracked the whole length of the block, by puting JB weld along the entire side of the block and spraypainting it black so it was undetectable until about 3 days

later when coolant started pouring out of the block. So I knew a good guy that had a full long block that had developed a slight knock then he pulled over and had it towed the mechanic told him it just needed bearings, So long story short I put rod bearings in it . took it out again and put new main bearings in it. (thrust bearing was toast) but all rods were tight on the wrist pins. crank and rods were all in good shape. i had the head off and put a new head gasket in and new head bolts.



I have tried cracking the injectors and it made no difference. I used the stethascope trick and narrowed it down to cylinder #4 as all 5 of the other cylinders run super smoothe but I can predominantly hear rough running in cylinder 4. I hear it in the head and on the block. cant decifer if its head or wrist pin.
 
If you have to go back into the engine I would mike the rod and main journals to make sure someone hasn't turned one of them undersize. Also, does the 01 still have the two piece tone ring on the crank like the 99? Another possible source of the noise could be the thrust surface on the crank could have been damaged by the previous failure causing the crank to walk back and forth. Good luck. bg
 
all the journals are good. the culprit ended up being a toasted wrist pin in #6 this tightened up the the piston to the rod and did not allow the piston to stay true in the cylinder and ultimately skirt started slapping the very bottom of the cylinder. There was was some very slight scoring in the cylinder way down low, completely out of reach of the piston rings, so a fresh hone, a new piston and rings this thing should be "on the road again" I will make sure to check the oilers and make sure they are not clogged.
 
She's alive!!!! I got the cylinder all honed out and put a piston from the engine I tore out of it with new rings. took my time and went through it slowly today. I took her for a test spin and she runs like a champ! damn that was 3 of the most grueling weeks of my life!!!
