My truck has recently developed a miss at low rpm. It will miss considerably more when the engine is cold but it also does it when the engine is warm. What would cause this illness.

Well your stock injectors have probably never been out and cleaned, and they have 278K on them. They are probably clogging up from carbon buildup! Try some injector cleaner first, it might not work though. Heck with that many miles the injectors might just be plain worn out. I just replaced mine at 245K, they were pretty much shot.
 
At 275k, if they've never been out, definitely remove them and have them cleaned, checked out, but at that many miles consider replacing them.



There are a number of things that can cause a miss. Start with the good basic maintenance of servicing the prefilter and replacing all the O-rings, and testing the preheater. Use an ohm meter, if the resistance is infinite it's bad, and sucking air. Sucking air can cause a miss. More info: http://dodgeram.org/tech/dsl/filter/pre-filt_clean.htm



It could also be the pump needs service, but start with the above. It could also need the timing checked, if it slipped that could cause a miss. Good luck.
