When removing the exhaust manifold should the bolts be loosened in a reverse of the install sequence? Also should they be nursed out as in the following vids?
 
not sure if its relevant, but on my 04 common rail I was worried about them but they all came out easy. I did not do anything special left one on each end to hold it up while I pulled them all and then put them back after resurfacing and replacing leaking gaskets. Stock manifold was decently warped so had a shop clean it up, no more leaks
 
I put the ATS on my ‘95. I don’t recall not being able to bolt the tubes up. This is the only pic I have showing it.

I have an ATS 3 piece and it has the mounting point for the heater hose mount. I suggest you call them to see if they have the wrong picture or changed the design.
 
The oem manifold on our truck has two heater line mounting points. One next to the # 1 and another next to #4. I anticipate a conversation with ATS in the morning. Thanks
 
I was hoping for a aftermarket manifold that accommodates the forward transmission cooler bracket as well. If there are no negative issues for the cooler without it great.
 
ATS Tech line says their 1-PC T3 manifold does not have any mounting bolt holes for the coolant lines. They said it was for the high performance applications, like racing etc..

I then called Geno's and asked for their recommendation, which is the BD brand. The kit offered through them does include gaskets, does not include the fasteners. They do sell stud kits however. With a military discount, shipping and tax, the cost from them s more than a couple other vendors that sell ATS and BD. Then also the sales are happening today.
