This comes up every so often and the simple answer is is that it doesn't work. There was a group of people at my school who were working on debunking different mileage improvements and figuring out which ones actually worked. To be complete, they tested the fitch and saw no difference.

There are ways to improve your mileage but there isn't a whole lot that can be done engine wise. On your engine, what is important is better airflow(intake and exhaust), better fuel atomization(injectors), and injection timing. The place where there is the most room for improvement is in vehicle aerodynamics. Assuming you want to keep the truck body, you can lower the truck and put a better front air dam on, but this significantly decreases your ground clearance.
