Red Loctite might be a good idea.
I had one installed on my '03. The shop didn't put anything on the threads, and they backed out. 3 of 4 loosened up. I didn't notice anything until one day I shut off my truck and PING! I heard something. I looked under the truck and found a bolt. After some searching I found out what it was.
Upon further inspection it turns out that the dampener had been vibrating around and the locating pin bent. I posted a thread about it a while back...
I sent the truck back to the shop and they reinstalled with red Loctite. No problems for 30k + miles. Truck is gone, and the new owner hasn't had an issue.
I'm looking to get one for my '07. I like how smooth it made the motor in nearly all conditions...
It came loose because they didnt install it right in the first place.
I never used loctite and hasnt fallen off
Really not too hard to install. Just take your time and get a barring (engine turning tool b series) from Snap-on (barring tool will assist you in locking your engine from turning over while your torquing the bolts).
Also use a torque wrench and take the bolts up to 150-165nm each.
Also install the belt on the damper before torquing/reassembling.

How do you know the install was not done right? Just because yours has not fallen off w/out locktite does not prove any thing.
When i had the dealer warranty my crankseal i watched the tech re install the pully w/ no problems. He said he used locktite but two days later 3/4 bolts loosened up. I reinstalled and saw no locktite on the threads. I applied w/ Locktite and no problems since.
