A sharp blow with a soft mallet (preferably Brass, or put some type of brass punch between a hard hammer and the lug) should pop the lugs out. It's a good idea to suspend the disc between cinderblocks or open a vise up on a bench, just don't want to be hammering on the back of the wheel bearing. It is recommened to replace the studs (although I'm sure many people don't) they're a buck or two each, pretty cheap insurance to hold your wheel on. Rotors can be had just about anywhere, Napa, RockAuto, Dealer even, just make sure not to get the cheapest Chinese ones out there, I've heard quite a few people like the slotted ones available at Napa. To reinstall the studs, put a couple washers against the rotor and use a lug nut to cinch the stud in tight.
