I have a video clip with my 2003 HO at idle. This noise has been present for a while, but I'm not sure what it is. I can't attribute the noise starting at any specific point or after any mod. I removed the belt and everything feels tight and spins free. The sound is a chirping or a sound like chains dragging the ground or maybe a tambourine. Frequency seems erratic and does not speed up or go away with higher RPM. It's a little hard to hear listening to the video, but if you have a Cummins, it should be pretty to pick out the dinging around 12 sec on the video. It's in the front of the engine, less noticeable on the side and nothing abnormal from the exhaust. I'm leaning toward tensioner, but I don't really want to start throwing parts. Any ideas or previous experience appreciated.

 
Could be the tensioner, could be the fan mount, could be injector rattle. Too hard to tell from the clip.
 
Did you run it (briefly) with the belt off? That will at least tell you if it is an accessory or if you have to look elsewhere.
 
May also be possible the impeller wheel on your water pump is coming loose and dinging around on thr block inside the pump housing. Would do a general check under and around the hood for any loose bolts or anything that could rattle around like that.
 
Mine made a noise like that. It ended up being the metal fan inside the alternator came off the shaft.
