I have a 93 w-250 with 170,000 miles on the untouched Getrag and clutch. I never did any heavy hauling and changed to Amsoil at 50,000. Over the last two years, I have noticed that when it is cold outside, that being below 60 here in Las Vegas, that it is very difficult to get it into any gear including 1st or reverse. When it is warm, there is no problem. I have changed oil, but that did not cure the problem. Is this a transmission problem or is there something wrong with the clutch not fully releasing when the pedal is down. I did have to replace the slave unit twice a couple of years ago. Otherwise, everything is factory.
 
If your amsoil is heavy, that is most likely the reason for stiff shift when cold. I use 85aw and I experience stiff shifting for about three miles. That is my first corner (stop) and as I shift up then, everything is fine. You could be having less than complete disengagment of the clutch and you might check all the points where there is a bit of slack. My feeling would be that if it is only when cold, it isn't a problem.

What weight of oil are you using in the trans? Some swear by using the thick stuff (80/90 wt. ), some by the thin (30 wt). Thinner the oil, easier the shifting, in cold weather areas.
 
I am using Amsoil 85/90, which I didn't think was too thick for 30-60 degrees. Since the Getrag takes motor oil, I may change it down to 40 weight. Thanks. Changing oil is sure cheaper than changing transmissions.
 
Lots of folkls tried a number of things before discovering that the Getrag need an extra qt of lube. You will get a stiff shift with that lube but thick or thin, be absolutely sure that you have the extra quart. Perhaps you already know about that but wanted to be sure.
