It could be the FCA, it could be the injectors, it could be crossover tubes and\or valves. Its hard to tell with minimal intermittent conditions.
You cannot run an additive once in a while, it has to be constant for it to do any good. Same with the extra lube. Since this ULSD is know for issues and is relatively dry consider a regimen of additives in every tank to rule out the effects.
I run a mixture of Power Service (96 oz) and 1 quart either 2 -stroke or 10 WT non-detergent oil in every tank, about 10 ozs per 1/2 to 2/3 tank fillup. The next best thing to do is hook a trailer on it and drive the snot out of it for 2 days.
