Engine/Transmission (1994 - 1998) Injector's coming loose

I was just wondering if any one has had or heard of the injectors rattling loose? Just had a little noise the other day and tonight on my way home it was really loud like exhaust leak. Opened hood and found number 4 injector was loose. Tightened up and retorqued all the other injectors.

Not on my Ram, but did on a John Deere 4430 tractor. #6 on that one. New washer on the bottom, and checked the torque on the rest. No problem in 7 years.
