I have the same problem. It started after pulling a backhoe 50 miles and up 7000 feet. It would happen when ever the RPM would drop below 600, starting or even when the engine would surge after sliping the trans out of gear when bracking to a stop. It started at 5000 mile, last april, but I have not noticed it for three months. I haven't changed my driving style so maybe it was the trans breaking-in.
 
Sounds like Flatudiesel Syndrome to me Matty. Too much methane held in suspension in your diesel. I heard it was caused by storing diesel in a tank that once held cow manure. Let it sit overnight and slowly loosen the fuel tank cap while sniffing at the opening.
