My 1992 W250 Clubcab w/auto trans and 87,000 miles is not revving out properly. In neutral I can only get 2700, and I used to get 2870 and when going in drive to overdrive it won't pull the revs it used to. This has been going on for about the last 1500 miles. I cleaned the air filter, changed the fuel filter and the throttle linkage is hitting the stops properly. My fuel mileage does not seem to be suffering, but I am. Any suggestions?

Sounds like maybe the throttle linkage itself is crappin' out? Is the linkage stock/factory? If so, maybe, the linkage plastic pieces are just getting worn out. I think actual age may have as much a factor on the plastic parts as the mileage. I don't have the pn# but the dealer sells brand new linkage piece, from the throttle cable lever to the pump lever, for about $30 I think. I replaced my linkage a while ago, and the new linkage has neoprene bushings and steel pivots, is absolutely bulletproof compared to the stock stuff, and I gained a no-kidding 20 mph because the pump lever was now traveling as far as my 'foot' thinks it should be. ... Might be the case... .
