My 1992 W250 Clubcab w/auto trans and 87,000 miles is not revving out properly. In neutral I can only get 2700, and I used to get 2870 and when going in drive to overdrive it won't pull the revs it used to. This has been going on for about the last 1500 miles. I cleaned the air filter, changed the fuel filter and the throttle linkage is hitting the stops properly. My fuel mileage does not seem to be suffering, but I am. Any suggestions?
 
Try checking the following items:

Make sure you are getting full throttle rotation by having someone depress the pedal to the floor while you look to see if the pump throttle plate rotates fully and contacts the high idle screw.

If not then you need to make some adjustments... many guys just get in the cab, and literally bend the gas pedal up a fair bit so it has more "throw". You can also look were the throttle cable protrudes from the gas pedal connector and put a spacer on the end of the cable so that some of the slack is taken up.

Next make sure that the locknuts on the high idle screw are snug. If they were adjusted or, if they have worked loose a bit you will some inadvertent movement of the high idle screw which may be keeping the throttle stop plate from pulling fully rearward at WOT.
