Send a check for $10 to me. Have the check made out to "GLTDR" and have your info written or typed on a piece of paper with the following info:

Name
Address
Phone Number
Truck info (year, 2 or 4 wheel drive, upgrades listed if you wish)
Email Address
Birth Date (if you want)
Wifes Name (if it applies and if you want)
Wifes Birthdate (if you want and/or if it applies)
