Engine/Transmission (1994 - 1998) More NV4500 Problems

Well I've posted in the past about my trans mission problems. In early April I broke the center out of my SB clutch> Its the 13" confe. I've been having problems since I had it installed (both times). Now it appears that I've broken another gear. Both failures accured while passing another vehicle. The clutch slips and then hooks (pretty violently) and thats when all hell breaks loose. What is going on? This is the 5th break in less than 2 years Any suggestions?
