How many miles are you getting on your NV4500? I don't tow heavy often, but it has happened. I don't have huge power (dynoed 290/790), and use it carefully, I also have the coolers hung on the sides. I have 286,000 miles, and am wondering if I should do the transmission as "preventative maintenance", or if I've made it this far will I be good for 500,000???

I would say you are better than most at that mileage. I had mine gone through at 180k as I was having problems with reverse due to previously losing 5th. Caused undue thrust wear and 5th gear was gone.
 
Broke 3rd gear in mine... ... ... ... less than your miles and never towed heavy. I also have the fast coolers. Normal oil change intervals.

185,000 on mine and have only changed the oil. About 330hp, but I am real easy on it in hopes of making it last. Have towed upwards of 30k from time to time and so far (cross my fingers) so good.
 
I have 229000 on mine without coolers and not a problem. I only have changed the oil once since 100000 with amsoil. On the same token, the only time I even remotely have a problem is when it's real cold, it's a little tempermental.
 
when I was looking at truck I wanted either a 5 or 6 speed, every 5 speed either had a bad 3rd gear syncro, had it replaced, or the dreaded 5th gear nut was either bad, or replaced.
 
Melted 3rg gear in mine (I tow about 23k combined most of the time) at about 125k. One thing I have learned after replacing the trans with all the 3rd and 5th gear upgrades is that the fully splined main shaft it not a fix all to the 5th gear falling off so don't fool yourself in thinking it can't happen after spending all that money.

I have 232k miles on mine, fluid ( R. P ) changed every year about 24-25k miles and have floated it every shift up or down towing or not. Still shifts like it did the very first day.
