I had a 95 with the nv4500.
I ran the bearings out of it in less than 80KM. In my case the truck was only modified by a governor speed limit change (not torque).
My owner manual said it had a life time synthetic oil but as others have found out, regular oil changes, perhaps every 50K or so might have prevented this problem. I had it rebuilt by standard transmission in Texas and had them put in a fully splined main shaft at the same time to help prevent 5th gear problems.
In spite of all this, it always shifted much nicer than my present Nv5600.

I have a 96 1 ton with a flat bed and the truck is loaded heavy all the time. It has a nv 4500. At about 140,000 it lost 5th gear. I had Standard Transmission send me a transmission with the fully splined shaft etc. The truck now has about 185,000 and I have had no issues at all. I think the nv4500 is a great transmission as long as you absolutely follow a couple of rules...

1. Absolutely positively over fill it with an extra quart of oil and it MUST be the correct oil. Use the Amsoil oil for the nv4500 or factory oil.
2. If you tow or are heavy CHANGE oil often.. I change it once a year regardless, I know that's probably overkill but its cheap insurance.
3. Try not to tow in 5th, at least don't lug it and pull hills etc.
