Thanks in advance on this one, it's got me puzzled.

Posting for a friend who can't get the truck to do this when I'm around, and he's legally deaf (He wears two hearing-aids, and can hear, but not that great), so I'm going on what he's told me.

On start-up (usually when already warm, but after sitting for half-an hour or more), it makes a slight "groaning" sound accompanied with a mild vibration that lasts only about a second.

Once when he had the hood open to try and diagnose, there was a small puff of white smoke from the passenger side of the engine.

Tuck has an older Bank's setup w/ "Big Hoss" unit.

Any advice?

Thanks!

I had a similar moaning and groaning sound it turned out to be the idler pulley. Picked up a replacement one at Autozone and 15 minutes later was back in business. The bearing and housing was really worn, my truck had @ 190K miles when I changed it.
