Need a Bushwacker Fender Flare

What does this mean? Could it be a vacuum leak? It's smoking a little but it's dripping oil every 10 seconds or so until it cools off.
 
I am the only one I have ever heard this happen to but, at the front of the block be hind the lifter cover is a freeze plug that caps the main oil feed coming up to the cam, and top end. on my truck the pushed out of the hole flooding the lifter cover and dumping oil out the case vent. the truck only had a 192K on it when it happened. its a lot of work but it might be worth the time to pull the cover just to check it out, that will be a lot cheaper then the $4400 it cost me to rebuild mine.
