Engine/Transmission (1998.5 - 2002) Power loss at 55 mph

Truck loses power between 50 and 60 mph. Power comes back if throttle released or if I turn key off and on quickly. Also will not go over 40 mph on cold start. After about 5 minutes engine runs fine. Installed new lift pump and new batteries. No help. Fuel pressure at test port by injector pump is 14 pounds while engine has no power. I checked it after cold start in the morning. Some white smoke with throttle wide open and engine at 2000 rpm in neutral.

Kinda sounds to me like a fuel filter. Has it been changed?

Yes the fuel filter has been changed.

No codes. I did take it in to the dealer. They kept it all day. When I picked it up that evening, they said they never had time to look at it. I have to have it to work.
