Alright this guy I know is selling a 99 2500 ext. cab 2wd and he has it listed for 5000 is this a good deal or am I wasting my time? thank you.

I Just Picked My 1999 Up About 30 Days Ago For Ten, But If The Truck Is In Good Shape And All Maintance Is Up To Date Then Yes It Would Be A Good Deal. Good Luck

If it is a guy you know you might have an idea what he used the truck for, and the care he took of it... I am not too scared of the high milage, more what it was doing during those miles...
If it were me, when I look at something like that I figure on needing to replace atleast one major item in the first few months/year (VP at minimum, figure a trans as average) then ask yourself if it is still a good deal. A big difference too if you can do your own work, or if you need to figure labor in also.
Things I would look for; where's it leaking, look under the hood if it is the original VP, check for bailing twine/wire holding anything together ;) ... If you have a mechanic you trusted and knows these trucks it would be well worth your money to have them do a once over on it.

Check block

It likely has the "53" block. Search here for "53" to see what to look for, namely cracking on the right side!

Ditto! I would not neccessarily not buy it because of this, but I would look it over extra careful and try to determine how well the truck was cared for and and how hard it was worked.

If you are still considering it,
I would have the mechanic check it over, and see if there is a big "53" stamped on the side of the block while he is there anyhow... not all '99s have the 53. Even if it does it only means there is a higher incidence of blocks cracking from that casting, it does not mean it will crack. kind of like a backwards lottery ;) just because you bought a ticket doesnt mean you will win the jackpot.
