Is it possible to replace the rear freeze plug in the block without pulling the head, and since it only leaks while driving, would a pressure relief kit help keep this from leaking?

They used to blow out on a small number of trucks and I was making and selling a plate to install and hold them in. If the trans is out you can probably sneak the plug out and back in.
 
Yes, you can get to it with the trans/bell housing out. I also made a plate to retain that plug. It bolts to existing threaded holes near the plug.
 
Sorry I was wrong, it's leaking on the back of the head either from the screw in plug, or the small freeze plug that are beside each other. I just got back from a 40 mile drive, and looked at it with a flashlight, and the back of the head is wet.
 
I found my old photos. The small plug is behind the adapter housing so I doubt that one is leaking. It should be the one that I made the tool to install and retain in place. It could also be the coolant tube that has to be moved to use the installer.
