When pulling away from dead stop there is a grinding sound coming from the rear end. Only lasts a few seconds. Any ideas as to the cause. I was thinking the gears, but just changed out the fluid a couple weeks ago (hoping to solve the problem) and they look good.
 
If you didn't see any metal debris when you changed the lube. I would drop the drive shaft and check the u-joints.



A fast easy check of the u-joints is to go for a test drive. 5 miles is good enough. Craw under the truck. A bad u-joint will be very warm to the touch. :D
 
If it turns out to be an internal problem, have someone hold their foot on the brake and drop it through forward and reverse with the cover off and see how much movement is going on, I had a spider gear doing the crazy dance in mine and it actually wore an egg shaped hole in the carrier and ground the cast iron down, I probably caught it just before catastrophic falure. Washers and bearings do wear out faster than gears and shafts
