i don't remember exactly but i had to replace one of my front wheel outer rings and i think it was around $100 from the dealer. i haven't used Phoenix stainless steel simulators before on a gmc and i think the set was around $150-200.
I ordered a set of J&J Enterprises and was impressed with the quality. Your 98. 5 should have four hand holes and you're in luck- that's the way they come. Nobody I know of makes an 8 hand hole model to fit the 2001 and up wheels. A good set of simulators will cost you a fraction of what you'd spend for the OEM plastic garbage.
I bought my AR simulators for $230 about 3 years ago. My only complaint about them is the centers on the rears are a pain in the ass to get off, i ruined one side because I couldnt get the center out. But I am a moron so i could have just put them on wrong or something... LOL. .
