Working on an 03 and this is the second time(different trks) I have seen the serpintine belt peel off the outside rib and continue on for thousands of miles. Its an 8 rib belt. Almost seems it is happy with a 7 rib belt.

Anybody seen this or have a remedy.

Is it an alignment issue.

More likely something is worn out and causing the belt to run off one side of a pulley or load up one side more. Have you checked the all the pulleys including the idler?
 
If alignment is the problem why does it only peel off one rib?? Could it have been installed with one rib off one of the pulleys? Just a thought. bg
 
Change the pulley tensioner assy and H2O pump

Just my 2 cents but for less trouble down the road ------>



If it has a lot of miles change the tensioner pulley assembly:



GATES 38285 TENSIONER ASSEMBLY ('03-'12, 5. 9L & 6. 7L)-Dodge Cummins Diesel Truck Accessories – Geno’s Garage



and check / change the waterpump and it's pulley:



DODGE DIESEL WATER PUMP ('98. 5-'07)-Dodge Cummins Diesel Truck Accessories – Geno’s Garage



It isn't that expensive and usually one of these is the culprit.

I had this exact issue on my first CTD ('92 model). It turned out to be the tensioner arm / pulley assembly. It was causing the belt to ride on the front of the A/C compressor pulley and cutting the outside rib off.



Change the complete tensioner assembly; not just the tensioner pulley.
