I was curious now with my truck having 234,000 on the stock injectors if there are any symptoms of wore out injectors. It seems to me that I don't have the bottom end power I should.
I'm no expert on this subject but I finally changed mine at about 387k and I picked up a fair amount of power and the truck produces less smoke for a given throttle position compared to the old ones. There is a very good chance yours are also tired. Mine still ran smooth and started fine.
Generally speaking, the pop-off pressure can fall, yielding less-than-desired atomization of the fuel, which can increase smoke and decrease power. Listen to the engine under load (going uphill, for example). If it sounds 'ratty', there's a fair chance the injectors needs attention.
Not long ago, I had the 215 injectors (Piers' 'slightly used') in my '98 (auto, 4x4, 19. 5" Hankooks) worked over. They'd been in there for around 150K miles. The engine had been sounding real ratty going up hills, and had a bit of a lope to it when idling. After the injectors were done, the engine was real smooth for a while. Now, the 'ratty' sound has returned a little. But with a double dose of Diesel Power cetane additive, I'm getting much better fuel economy. On a recent trip, empty, from Elgin, IL, back to VA via Ann Arbor, I got 19. 7 MPG, including an 85-90 MPH sprint across I-94 in MI. Musta had a good tailwind that day.

A friend that's a diesel injection mechanic told me 150k is a good time to pull & check 'em and shouldn't go more than 200k. On both of my trucks when I pulled the original injectors they looked really bad, tons of buildup on the tips. Since 12v motors seem to carbon up tips more readily than 24 valves I'd say it's worthwhile to pull & clean tips at least every 100-150k.
